Sample Fill and Print Acrobat Form The sample fill and print form was created by printing the word document to an "Acrobat Printer"and then using Acrobat 9.0 and the form wizzard. Only a few fields had to be changed (eg. The Yes/No options were changed to be check boxes and set up so that you could only choose one response.)
